Wound Care Management

Hands-on wound assessment, dressing changes, and debridement in our Middletown office or at home — for post-surgical incisions, pressure injuries, and slow-healing ulcers.

Who it is for

  • Post-surgical wounds that need regular dressing changes or monitoring
  • Pressure injuries, including Stage III–IV
  • Diabetic and venous ulcers that are slow to heal
  • Patients for whom a weekly trip to a wound care center is not realistic
  • Self-referred patients, and referrals from surgeons and hospital wound centers

What a visit covers

  • Full wound assessment and a personalized healing plan
  • Routine and complex dressing changes
  • Debridement when appropriate
  • NPWT (wound vac) management
  • A healing plan coordinated with your surgeon

Types of visit

  • Initial assessment — wound assessment and care plan, one time
  • Standard visit — routine dressing change, stable wound
  • Complex visit — Stage III–IV, multiple wounds, debridement, NPWT
  • Available as a stand-alone service or alongside chronic disease and transitional care
What happens next

A callback within 48 hours to scope the wound, how often you need to be seen, and whether an initial assessment is the right starting point.
