About Us
Tikari is a nurse-practitioner-led practice serving adults across Delaware and Maryland, founded and run by Jerise Agha, MSN, AG-ACNP-BC.
Quality Care That Meets You Where You Are.
Tikari combines office visits, in-home care, and virtual access — so following up fits your situation instead of the other way around, and nothing falls through the cracks in the first critical weeks.
Your first visit & care plan
A comprehensive intake — full history, medication reconciliation, and an individualized care plan built around your conditions and goals.
Stay connected virtually
Scheduled telehealth visits and messaging with Jerise for questions, medication concerns, or new symptoms — no waiting weeks for a callback.
Get seen in person when it matters
Hands-on assessment in our Middletown office, including wound care such as dressing changes and healing checks. When getting to an appointment isn't realistic, we come to you instead, removing the transportation barrier that keeps sick patients from following up.
- Adult-Gerontology Acute Care Nurse Practitioner, board-certified

ICU-level clinical judgment, applied to outpatient chronic care.
Jerise Agha, MSN, AG-ACNP-BC, provides coordinated chronic disease and post-hospital care for adults with complex medical needs.
Drawing on extensive hospitalist and critical-care experience, she focuses on individualized care plans, medication support, early recognition of worsening symptoms, and smooth coordination with specialists and hospitals.
Acute-care pedigree. Confidently manages higher-acuity patients than a typical generalist family practice.
Built around the discharge window — the first 30–90 days after a hospital stay, when care needs change fastest.
Hybrid access. Office visits, in-home care, and virtual follow-up — so transportation never decides whether you get seen.
